First part: Identification of Beneficial Owner

In this section, it is necessary to fill in the following fields:

2️⃣ Country of birth.
3️⃣ Residential address, indicating city, province, postal code, and country.
4️⃣ This section will only be completed in the event that the address for notifications is different from the fiscal address.
5️⃣ It is not necessary to fill it out (If prompted for information, you can enter a period).

6️⃣ Item A. National identification number (DNI or NIE).
      Item B. Check the box.
7️⃣ It is not necessary to fill it out (If prompted for information, you can enter a period).
8️⃣ Date of birth.

 

Second part: Claim of Tax Treaty Benefits

Here you only need to fill out point 9, indicating your country of residence. Point 10 is for special cases, and in the event it asks for information, you can fill it in with a period.

 

Third part: Certification

In this section, you'll need to:

1️⃣ Check the box certifying your ability to sign the document.
2️⃣ Perform the digital signature.
3️⃣ Enter the date on which you're signing the document.
4️⃣ Type your name.
