Guide to completing your Live UProfit account agreement

Follow these instructions carefully to complete and sign the agreement correctly in your Live account ✍

1️⃣ Carefully read the information contained in the agreement.
2️⃣ After reading the agreement, it will be necessary to fill in the required fields. Here's how to complete each field in order:

πŸ–Š On the first page, you'll find the following fields:

Enter the corresponding information for each field according to its number:

1. Your name.
2. Date on which you are signing the agreement.

πŸ–Š Subsequently, on the last pages of the agreement, you'll find the following fields:

Remember to enter the corresponding information for each field according to its numbering:

3. Your full name.
4. Your residential address.
5. City.
6. State, province, department, or region.
7. Country.
8. Your postal code.
9. Your phone number.
10. Email address (must be the same one registered with your UProfit account).
11. Your signature.
12. Your date of birth.
13. Attach proof of residence: it can be a recent bank statement or summary or a recent utility bill for energy, internet, mobile, or telephone in your name indicating your current residential address. This document cannot be in someone else's name and must be a complete document.
14. Attach a selfie holding your ID: this photo must be a selfie holding your identification document, the information on said document must be legible, and the photo cannot be in mirror mode.
15. Attach an image of your identification document: this document can be your national ID card or valid passport. Attach an image of both sides of the document.

3️⃣ If you are a resident of the United States, you should have received an agreement including a W-9 form. If you are not a resident of the United States, you should have received an agreement including the W-8BEN form.

πŸ–Š If you need to complete the W-9 form, complete it as follows:

The form must be signed personally/individually (cannot be signed as a company or business). Complete only the following fields (it is not necessary to enter information in fields not mentioned below):

Field 1 (Name): Enter your full name.
Field 3: Mark with an 'X' the option 'Individual/sole proprietor or single-member LLC'.
Field 5 (Address): Enter your complete residential address.
Field 6 (City, state, and ZIP code): Enter your city, state/province, and postal code.
Part I (Taxpayer Identification Number): Enter your social security number.
Part II (Certification):
Signature of U.S. person: Enter your signature.
Date: Enter the date on which you are signing the form.

🚨 IMPORTANT: In case the application asks you to enter information in fields that do not need to be completed, please complete them with a period '.' or writing 'N/A'.

πŸ–Š If you need to complete the W-8BEN form, complete it as follows:

Complete only the following fields (it is not necessary to enter information in fields not mentioned below):

Part I (Identification of Beneficial Owner):
Field 1: Your full name.
Field 2: Your country of nationality.
Field 3: Your residential address, including city, state/province, and country in the corresponding fields.
Field 6a: Your national identification number (DNI or NIE).
Field 6b: Check the box.
Field 8: Your date of birth.
Part II (Claim of Tax Treaty Benefits):
Field 9: Your country of residence.
Part III (Certification):
1. Check the box.
2. Enter your signature.
3. Enter the date on which you are signing the form.
4. Write your full name.

🚨 IMPORTANT: In case the application asks you to enter information in fields that do not need to be completed, please complete them with a period '.' or writing 'N/A'.
